Post-Operative Instructions After Total Thyroidectomy

1. Wound Care

Keep the neck incision clean and dry for the first 48 hours.

After 48 hours, gentle showering is allowed, but do not scrub the wound.

If steri-strips are present, allow them to fall off naturally.

Watch for:

Increasing redness

Swelling

Pus discharge

Fever

If any of these occur, contact your surgeon immediately.

2. Neck Care and Activity

Mild neck stiffness is common.

Avoid:

Heavy lifting (>5 kg)

Strenuous exercise

Sudden neck movements

for 2 weeks.

Gentle neck stretching exercises may begin after 5–7 days if comfortable.

3. Voice Changes

Temporary hoarseness or voice fatigue may occur due to irritation of the Recurrent Laryngeal Nerve.

Seek medical advice if:

Voice becomes progressively worse

Difficulty breathing or swallowing develops

Voice does not improve after 2–3 weeks

4. Calcium Monitoring

After thyroid removal, calcium levels may fall due to temporary dysfunction of the Parathyroid Glands.

Symptoms of low calcium include:

Tingling around mouth

Tingling in fingers or toes

Muscle cramps

Hand spasms

If these occur, contact your doctor immediately.

You may be prescribed:

Calcium tablets (Shelcal 500 mg in tapering doses)

Or Calcitriol occasionally

Take them exactly as advised.

5. Thyroid Hormone Replacement

Because the thyroid gland is removed, you must take lifelong thyroid hormone replacement.

Typical medication:

Levothyroxine ( Thyronorm) ask your doctor for exact dose

Instructions:

Take once daily in the morning

On an empty stomach

30–45 minutes before food

Do not miss doses.

6. Diet

Normal diet can be resumed.

Soft foods for the first 1–2 days may be more comfortable.

Drink plenty of fluids.

7. Scar Care

Once the wound heals:

Use sunscreen or scarf for 3 months to reduce scar darkening.

Gentle scar massage after 3 weeks may improve cosmetic outcome.

8. Follow-Up

Follow-up is important to monitor hormone levels.

Typical schedule:

First review: 5–7 days

Thyroid function test: 4–6 weeks

Periodic follow-up thereafter.

Tests may include:

Thyroid Function Test

Serum Calcium Test

9. When to Seek Emergency Care

Go to hospital immediately if you develop:

Neck swelling or tightness

Difficulty breathing

Persistent vomiting

Severe tingling or muscle spasms
